Immunization Form, CPR, and Criminal Background Check Information


Clinical Health Clearances for Nursing Students

Prior to starting classes, all undergraduate and graduate nursing students must receive health clearance in order to meet both the university and clinical healthcare agency requirements. Failure to meet any of these requirements will jeopardize your ability to attend clinical courses.

Please note that the forms and instructions for the 2008-2009 academic year have changed!
Read all of this information carefully to ensure that you meet all requirements by the start of classes.

Immunizations

All undergraduate and graduate nursing students must submit a complete immunization record on entering Georgetown, and verification of a renewed PPD annually. Particularly, remember:

  • Hepatitis B is required in addition to the usual childhood immunizations. Hepatitis B immunization consists of a series of 3 injections over 6 months. The immunization form should reflect that you have begun this process by the time it is returned.

  • A PPD is required annually; new students must have a two-step PPD as of 2008.

  • A Varicella vaccination or a Varicella blood titer (chicken pox) is required. To have had the chicken pox as a child may not be not be sufficient protection against the disease. A titer is therefore necessary to prove immunity.


Physical Exam

All undergraduate and graduate nursing students must have an annual physical exam, and your healthcare provider must sign the Student Nurse Annual Wellness Form to verify that you are in good health and can
participate in the clinical setting.

There are separate Annual Wellness forms for new and returning students. Returning students should submit this form to the Office of Academic Affairs; new students should submit this form directly to the Student Health Center.


Drug Screening

All entering undergraduate students in 2008 are required to pass a urine drug screen. This includes all first year, transfer, and new second-degree nursing students. *The urine test must screen for amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine metabolite, ethyl alcohol, methadone, opiates, phencyclidine, propoxyphene, and tricyclic antidepressants.* Your general physician or the Student Health Center can conduct one for you, and verify the results on the New Student Nurse Annual Wellness form. New students should submit this form in conjunction with your immunization form directly to the Student Health Center.


CPR (BLS for Health Care Providers) Certification

A Basic Life Support (BLS ) cardiopulmonary resuscitation (CPR) course specifically for health care providers (not lay people) is required, and must be kept up-to-date at all times.

Now it's simple to complete this requirement! Several training sessions will be offered right here in St. Mary's 107 on August 25 & 26, 2008. These courses will be provided in conjunction with Georgetown University Medical Center to all first year and new second degree BSN students, as well as those current students who need to renew their certification.

No need to change your summer plans, travel across town, or scramble to find a training facility the day before clinical starts! Instead, plan to attend one of the following free sessions on campus before classes begin next fall:

Initial Certification August 25, 2008 8:00am - 12:30pm
Initial Certification August 25, 2008 1:00pm - 5:30pm
Initial Certification August 26, 2008 8:00am - 12:30pm
Renewal Certification August 26, 2008 1:00pm - 4:00pm

Criminal Background Check

In addition to the above, one more requirement must be completed before you enter the new school year in the fall.  All nursing majors must secure a criminal background check every year.  The company that the School uses is CertifiedBackground and the only thing that you will need to do is to go to http://www.Certifiedbackground.com and click on “Students”.  In the package code box, enter the package code EO14 (please note it’s an “o” in EO14, not a zero).
